  2. Please make the payment for $415. $15 is the credit card processing fee. $400 will go towards your surgery and will be deducted from your final payment.
  3. If canceled more than 30 days from the date of your surgery, $200 of this down payment will be returned to you (minus any processing fee that your credit card company would charge us).
  4. This fee is not refundable if canceled within 30 days of surgery.
